Privacy notice

How Dr Danie Hoffmann's practice collects, uses and protects patient and website visitor information under POPIA and HPCSA rules.

Who we are

Dr D Hoffmann Inc. (Reg No 2005/039204/21, VAT 4910263120), trading as Dr D Hoffmann, of 3 Miervreter Road, Van Riebeeck Park, Kempton Park, 1619, is the responsible party for personal information collected through this website. Practice No 3601358 · HPCSA No 261190.

What this website collects

  • Enquiry form: your name, email address, telephone number, the procedure you are asking about, your message, whether you consented to being contacted, and whether you opted in to practice updates.
  • Technical information: the page the enquiry was sent from, and a one-way hashed form of your IP address used only to limit abusive submissions. The raw IP address is not stored.
  • Analytics: anonymous usage statistics, described in the cookie notice, which you can decline.

The website does not ask for clinical details, and you should not send medical history, images or identity documents through the form. Please raise those in consultation.

Why we use it

  • To respond to your enquiry and arrange a consultation.
  • To send you a confirmation that your enquiry reached the rooms.
  • To keep an administrative record of enquiries received.
  • To send practice updates, only if you ticked the optional opt-in — you can withdraw at any time.

Who processes it on our behalf

Enquiries are stored and delivered by the service providers listed in the sub-processor schedule. They act on the practice's instructions only and may not use your information for their own purposes. Your information is not sold, and it is not shared with anyone else without your consent, unless the law requires it.

How long we keep it

Website enquiry records are kept for 5 years and then deleted. Clinical records created once you become a patient are held separately and retained for the periods required by the HPCSA and the National Health Act.

Security

Enquiries are transmitted over an encrypted connection and stored in an access-controlled database that is not readable from the public website. Access is limited to the rooms. No system is perfect; if a breach affects your information, you will be notified as POPIA requires.

Your rights

You may ask what we hold about you, ask for it to be corrected or deleted, object to processing, or withdraw marketing consent.
